Amergis Aging and Disability Services - Intake Coordinator in Caribou, Maine

Overview :

Temporary Intake Coordinators will assist individuals/families in applying for OADS eligibility for services. These staff will meet with individuals and families at a place of their choosing, to assess the individual’s needs and family dynamics. They will explain important requirements and timelines associated with the process. These staff will also provide an explanation of services that will be available if the individual is found eligible, and the individual's right to appeal if found ineligible. In addition, Temporary Intake Coordinators will collect important information related to eligibility such as school records, psychological reports and assessments, and functional assessments. They will enter all necessary data into Evergreen and then share pertinent information with Intake Supervisor, who will then make a final determination as to the individuals' eligibility for OADS services.

Pay Rate: $30/ hour, 40 hours/week

13 paid holidays per year

Up to 10 days of accrued PTO, based on assignment duration

Please note that this pay range represents a good faith estimate of the compensation that will be offered for this position. The actual pay offered to a successful candidate will take into account a wide range of factors, including but not limited to location, qualifications, and additional preferred experience.

Knowledge, Skills and Certifications:

Minimum Requirements for position: Bachelor’s Degree

  • Knowledge: Must hold a Bachelor’s Degree in a related field, have two years of experience working with individuals with Intellectual Disability or Autism. Should have in depth understanding of OADS and the services it provides.

  • Skills: Ability to communicate effectively, and listen intently. Ability to document and summarize service needs. Should be familiar with Microsoft Office products, will input necessary data into Evergreen system. Should be comfortable meeting with families and individuals served, in public or private settings.

Duties and Deliverables:

  • Schedule and meet with individuals or families interested in applying for OADS services.

  • Document the Individual needs and family dynamics, outline the Intake "process" with included timelines and legal right to appeal a final determination.

  • Describe OADS available services, get a signed consent for treatment, and assist the individual or family, as needed, in arranging for the gathering of additional psychological records.

  • Enter all data into Evergreen and complete any remaining Intake requirements as directed by the Supervisor. There will be limited local travel.

Schedule: Routine business hours, Monday-Friday (40hrs/week, Limited local travel.
